Qualifications
· Current Certificate of Competence from the College of Nurses of Ontario and current PLP.
· Bachelor of Science in Nursing is an asset.
· Strong working knowledge and proficiency in relevant computer applications (i.e. Microsoft Office: Word, Excel, Outlook, nursing care plan software applications, i.e. Point Click Care etc.).
· Leadership experience and extensive knowledge of care requirements for seniors in long-term care settings.
· Customers’ services experience is an asset.
· Demonstrates a strong sense of leadership, team building, interpersonal dynamics, and problem solving including conflict resolution skills.
· Current CPR.
· Fully vaccinated with approved COVID-19 vaccine & willing to adhere to IPAC recommendations with the any following vaccines.
· Ability to communicate in the Finnish and/or Estonian language is an asset.
· Must be able to motivate and direct the care team.
· Must relate to people of all ages and levels, both professional public.
· Must have sound critical thinking and resource management skills.
· Must be able to commit and achieve repeated deadlines.
· Excellent verbal and written communication skills.
Responsibilities
1. Provides clinical oversite to care staff (e.g. RPNs, PSWs) during shifts, to ensure that the residents’ needs are met and unit activities carried out.
2. Facilitates resolution of care staff issues, including intervening and reporting of interdepartmental conflicts that may occur during the shift.
3. Ensures that appropriate action is taken for work-related injuries and other employee incidents that occur during the shifts. Informs management of incidents as soon as possible.
4. Ensures ADOC/EDOC is notified on any performance or safety issues in writing.
5. Replaces care staff, where necessary, in the absence of the scheduling clerk.
6. Authorizes shift exchanges that are urgently needed.
7. Seeks direction from ADOC/EDOC for the authorization of overtime.
8. Provides input to the EDOC into the acquisition or purchase of supplies and other equipment needed for resident care.
9. Ensures that appropriate action is taken for equipment failures during the shift.
10. In the absence of clerical support, completes or ensures that the appropriate clerical activities are performed.
11. Communicates relevant information to oncoming care staff through a taped report, where required.
12. Completes the section relating to her shift on the 24-hour report and submits the completed report to the ADOC/EDOC on the following day.
13. Establishes regularly scheduled MKK Unit meetings with care staff meetings during the shift and chairs the meetings.
Nursing
1. Assesses, plans, implements and evaluates care provided by him/herself and other staff to ensure that standards of nursing practice, the MOH LTC’s guidelines and OFRA’s policies and procedures are maintained.
2. Completes the admission profile of residents and the discharge of a resident.
3. Ensures that an interdisciplinary care conference is scheduled annually for each resident and chairs the meeting. Also, schedules additional care conferences to address specific resident-related issues, as the need arises.
4. Assesses the need, on a daily basis, for continuing to restrain identified residents. .
5. Ensures that appropriate action is taken for any resident-related incident (e.g. injuries, medication errors) that occurs on her shift.
6. Acts as a liaison between the residents, their physician and other providers (e.g. physiotherapist, dietician, external clinics, etc.) Assists the attending physicians during their visits with residents.
7. Manages infection control issues on her/his shift and ensures that the appropriate action is taken on the unit following the issue.
8. Acts as a liaison between the residents and other OFRA services (e.g. food services, laundry and housekeeping services). Refers issues to the ADOC/EDOC, where necessary.
9. Facilitates resolution of resident/family conflicts and issues. Refers the issues to the ADOC/EDOC, where necessary.
10. Ensures accurate and complete documentation by care staff.
11. Completes accurate resident records and other reports (e.g. scheduled chart reviews, death reports) in accordance with the standards of nursing practice, MOHLTC’s guidelines and the policies and procedures of the OFRA.
12. Ensures that relevant practitioners complete annual physicals, 3-month medication reviews and quarterly summaries.
13. Reviews and cosigns the new MAR sheets after the RPN’s initial check.
14. Ensures that registered staff complete the narcotic count at the beginning and end of each shift and witness the drug destruction process.
15. Provides PRN health teaching to care staff about the residents’ health conditions and needs to ensure that staff takes an appropriate approach with the residents.
16. Participates in Nursing practice meetings.
17. Maintains regular communication with the ADOC/EDOC, including consultation with the EDOC on issues that affect staffing, or that pose a risk to the organization (e.g. resident safety)
18. Continue his/her professional development, as per College of nurses of Ontario quality assurance standards, through continuing education opportunities.
19. Other duties as assigned by the ADOC/EDOC.
